2015-16 Stakeholder Survey

I would like to make everyone aware of an upcoming school district survey or more commonly known as stakeholder input. The purpose of the survey is to gather valuable information from the stakeholders in our district. Parents and guardians who have students in 3rd through 12th grades are encouraged to participate in the survey. The surveys are given to the classroom teacher and building administrator to evaluate and identify the strengths and deficiencies in the classrooms, schools, and district. Please know, the responses to the survey are anonymous and no personal information will be collected from you when you conduct the survey. The survey is short and will take less than 10 minutes to complete. If you have multiple students in the district or if have a middle or high school student, you can complete surveys on any or all of their classroom teachers. You will be able to access the survey beginning January 20th through February 10th on the district web page at http://www.garfield.k12.ut.us . If you have questions regarding the surveys or if you need help in any way, please contact us at the district office at 435-676-8821. I would like to thank you in advance for your participation.
